For most people, going to the dentist is as routine as getting their oil changed. But if you’re like the one in ten people with severe anxiety, dental visits are anything but routine.
What may have begun as a childhood fear has turned for many people into a lifetime avoidance of dental care. Â This absence of dental cleanings, checkups and treatments can have an adverse effect on not only their oral health but their general health too.
But there are ways you can reduce dental visit anxiety, beginning first with finding a compassionate dental provider. A good dentist-patient relationship is important for everyone, but more so for people with anxiety. Building a trust relationship with a dentist who listens and accepts your fears without judging is your first step to overcoming them.
Though finding an understanding provider is important, it may not be enough in the beginning of your return to regular dental care. To help you further relax during visits, we can also provide medicinal therapies known collectively as sedation.
Although it has some similarities, sedation is different from anesthesia. The latter deadens pain sensation; sedation aims to calm your emotions. The most common sedation is taken in oral form, usually a pill (or syrup for children) taken an hour or so before the appointment. Oral sedation is often used in conjunction with gases like nitrous oxide and local anesthesia.
For a more relaxed state (especially during an involved procedure) we may use intravenous (IV) sedation. With this method we deliver the medication through a small needle or catheter inserted into a vein.
IV sedation places you in a reduced state of consciousness. But it isn’t a “sleep” state as what’s achieved during general anesthesia, but more of a “semi-awake” state. You won’t need assistance with breathing or heart function and you can respond to verbal or touch commands. Many drugs used for IV sedation also have an amnesiac affect, so you won’t remember many details about the procedure.
Depending on your level of anxiety, we can match the right therapy to induce calm and relaxation. Sedation can help you see dental visits in a more positive light so that it truly does become a life routine.

Visiting the dentist for cleanings, checkups and needed dental work is one of the pillars of dental health, along with daily hygiene and a nutritious diet. But an estimated 50% of people have some form of anxiety about dental visits — and around 15% actually avoid care because of it.
If you feel nervous about dental visits, there are ways to reduce your anxiety. First and foremost is to find a compassionate provider you trust and feel comfortable around, who listens non-judgmentally to your concerns.
But that’s only the beginning: depending on your degree of anxiety, you could require more help to relax through sedation medication. The drugs and methods used can induce various degrees of consciousness ranging from mild relaxation to more sleep-like states.
The most basic is oral sedation. Typically, this involves taking the medication by mouth about an hour before an appointment. You can take it by itself to increase relaxation or along with other forms of sedation (like inhaling nitrous oxide gas) or local anesthesia.
Beyond inhalation, a higher level of sedation involves injecting the medication into the blood stream through an intravenous (IV) drip. This induces a deeper “semi-awake” level of consciousness, but differs from general anesthesia, which places a patient into unconsciousness to block pain during a major procedure. With IV sedation you may still be able to respond to verbal commands or touch; and although you’re monitored for vital signs you won’t need medical assistance to maintain breathing and heart function.
With today’s advanced sedation drugs and methods, we can control dosages to achieve just the right level of sedation, as well as reduce the amount of time the drug may affect you afterward in recovery. Many drugs also have an amnesiac effect so that you’ll remember little if any about the procedure afterward.
Whether by mouth, inhalation or with an IV, sedation therapy can make a difference no matter what your level of anxiety. And if your dental visits continue to be comfortable and pleasant ones, you’re more likely to receive the care you need to keep your teeth and gums healthy.

The next time you’re visiting Boston, why not make time for a stroll in the city’s renowned public garden? It’s got a little something for everyone: acres of greenhouses and formal plantings, a picturesque pond where you can go for a paddle in swan-shaped boats, and the first (and perhaps the only) statue dedicated to an anesthetic gas.
Yes, the Ether Monument (also called “The Good Samaritan”) is a vaguely Moorish-looking sculpture that commemorates the first use of anesthetic in a medical procedure. This ground-breaking event took place at nearby Massachusetts General Hospital in 1846. But if it seems that perhaps the park designers were feeling a bit light-headed when they commissioned this statue* then just think of what it would have been like to have a tooth drilled without it!
Today, of course, ether is no longer used for anesthesia; that’s because medical science has developed far better ways to make sure you don’t feel pain when you’re having a procedure. However, we do still use a gas for people who need a little more help relaxing during dental treatment. It’s called nitrous oxide, but sometimes goes by the nickname “laughing gas.”
This sweet-smelling gas, mixed with oxygen, is often administered in a process called inhalation conscious sedation. It doesn’t put you to sleep — you can still follow directions and respond to verbal cues — but it makes you very comfortable, and may even induce a slightly euphoric feeling, which wears off quickly when the gas is stopped. That’s what makes it ideal for some dental procedures: It’s quite effective for people who might otherwise have a great deal of dental anxiety, yet it’s quick, easy and safe to administer — and you can usually drive yourself home afterward.
Sometimes, however, you may need even more relaxation — for example, if you’re having multiple wisdom teeth extracted. In this case, it may be best to use intravenous (IV) conscious sedation. Here, the precise amount of medication you need is delivered directly into your bloodstream via a tiny needle. As with nitrous oxide, you’ll remain conscious the whole time, but you won’t feel any pain — and afterward, you probably won’t remember a thing.
